Methodology Conventional Culture 
Description Includes Gram stain. Significant isolates reflex to organism identification and antibiotic susceptibility test. Additional charges apply. 
Specimen To verify appropriate test, refer to culture for specific specimen source, if listed.

1. Aseptically collect specimen from affected area.
2. Submit specimen in a screw-capped, sterile container or swab in a culture transport media tube.
3. Label container with patientís name (first and last), date of birth or medical record number, date and time of collection, collectorís initials and specimen source.
4. Maintain sterility and forward promptly

NOTE: SPECIMEN SOURCE IS REQUIRED ON REQUEST FORM FOR PROCESSING.
