Methodology Enzyme-Linked Immunosorbent Assay (ELISA) 
Description Useful for evaluating patients suspected of having celiac disease, including patients with symptoms compatible with celiac disease, patients with atypical symptoms, and individuals at increased risk of celiac disease. Also useful for evaluating the response to treatment with a gluten-free diet. 
Specimen Draw blood in a plain red-top or serum gel tube. Spin down and send 0.8 mL (minimum: 0.5 mL) of serum refrigerated in a screw-capped, plastic vial. If specimen will not arrive within 2 days of draw, send specimen FROZEN in plastic vial on dry ice.
Reference Values Negative: < 20 U
Weak Positive: 20-30 U
Moderate to Strong Positive: > 30 U

According to World Gastroenterology Organization Practice Guidelines on celiac disease, antigliadin antibody assays have low sensitivity and specificity, and relatively poor positive predictive value in the general population. The recommended serology test for celiac disease diagnosis is the IgA anti-tissue transglutaminase antibody (celiac disease panel CDP).
