Methodology Liquid Chromatography / Tandem Mass Spectrometry (LC/MS/MS)) 
Specimen SUBMIT ONLY 1 OF THE FOLLOWING SPECIMENS:

PLASMA:
Draw blood in a green-top (sodium heparin) tube. (PLASMA GEL TUBE IS NOT ACCEPTABLE.) Spin down and send 1.0 mL (minimum: 0.6 mL) of plasma refrigerated in a screw-capped, plastic vial.
NOTE:
1. Label request form and specimen as plasma.
2. Trough levels are most reproducible.

SERUM:
Draw blood in a plain, red-top tube. (SERUM GEL TUBE IS NOT ACCEPTABLE.) Spin down and send 1.0 mL (minimum: 0.6 mL) of serum, refrigerated in a screw-capped, plastic vial.
NOTE:
1. Label request form and specimen as serum.
2. Trough levels are most reproducible. 

Reference Values Encainide:
Reference range: 15 - 100 ng/mL
Critical value: 2500 ng/mL

O-Demethylencainide (ODE):
Reference range: 100 - 300 ng/mL
Critical value: 1200 ng/mL

3-Methoxy-ODE (MODE):
Reference range: 60 - 300 ng/mL
Critical value: 1500 ng/mL

Ten percent of patients do not form therapeutic concentrations of the active metabolites, ODE and MODE. In these patients the recommended range for the encainide concentration is 300 - 1200 ng/mL.
