NOTE:
1. Third party billing IS NOT AVAILABLE for this test. When ordered on Medicare patients, a properly executed Advance Beneficiary Notice (ABN) must be submitted with the specimen.
2. Performance characteristics have been determined by the performing laboratory. Results should be interpreted in conjunction with clinical findings. 

Specimen 1. Prechill a 5.0-mL lavender-top (EDTA) tube at 4 C before drawing the specimen.
2. Draw the prechilled lavender-top (EDTA) tube, and process 2.0 mL (minimum 0.5 mL) of EDTA plasma from a fasting patient as follows:
A. After drawing specimen, chill lavender-top (EDTA) tube in wet ice for 10 minutes.
B. Centrifuge in a refrigerated centrifuge or in chilled centrifuge cups.
C. Immediately after centrifugation, remove EDTA plasma, place in plastic vial and freeze.
3. Send specimen FROZEN.
